Referral Procedure

So, how does one with an intellectual disability get a residence with us? Please follow our referral procedure below to be placed into our residential care. 

Referral procedure steps:

1. Firstly, the individual being referred to our residential support and care will need to be diagnosed with an Autistic Spectrum Disorder and/or an Intellectual Disability by a District Health Board (DHB).
2. The parent or guardian will need to discuss the need for residential care with the NASC.
3. NASC will either make a referral directly with us or you can fill out our referral form here and email it to admin@avonsidehouse.org.nz or post it to our PO Box 32-156, Christchurch 8147.
